That sounds very odd. I think you're going through the right things in your checklist. I'd also think very hard at what happens on minute 21 of you leaving the machine alone. Power spikes ? Do you have a UPS ? Is it a coincidence you upgraded and this started or can you reproduce this on another similar system nearby ? This may sound obvious but make sure your system has all updates applied.
