What really worked for me was dividing a paper into 20
sections (one for each student.  I printed each
student's name into each section (one section per
student).  Since each section was about 2"by 2" there
was plenty of space for comments.  I had one sheet per
week and tried to keep up.  For the upcoming year I am
going to bind 40 sheets so that I can keep the booklet
together and it will be easier to keep track.  
I plant to have one for writing and one  for
math----and try to take more notes.
