Jen,

     I teach first grade also.  Would you be willing to share the forms you 
used during your guided reading groups?  Is there anyway that you could adapt 
your 1st grade forms to fit your new assignment?  I realize that you will be 
more "comprehension" focused in 3rd grade.  Maybe you could use the strategies 
in "Mosaic of Thought" as your basis.  I am using the CAFE Assessment this year 
in my class in conjuction with the Daily Five.  The "CAFE" book speaks of also 
forming strategy groups-- not just leveled groups like I've done in the past.  
You might want to check out this resource as well.  We are beginning with RTI 
this year, and I know that I will need to be more diligent with my record 
keeping.  Good luck!
