Sorry, I sent this message off in a hurry.  To clarify, I have notebook
paper for each student in my assessment notebook, and that is where I stick
the labels.  Each student has his/her own tab section.  It is very useful
for planning and conferences.

The groups are flexible (though with RtI pullouts, it is becoming a
challenge), and the section for groups is planning page forms and notes from
the group as a whole when necessary.
