I just keep a regular 1 subject spiral notebook.  I use the sticky tabs to
divide groups...leaving a few empty pages between each group.  We always
start with "shared reading" a poem together and I make a little grid on top
with students names down the side (date on top) so I make sure each day
another child gets to pick the poem.  (I give them 1 poem every week to keep
in their folders)  Then underneath the grid I write the date, book we are
reading and then take notes as necessary.  I may only write "mary"-choppy
reading 
Fred- tried beginning of unknown word before asking for help Greg- asked a
good question, I will write that down.  I don't have notes on every child's
reading every day.  I can easily move the child to another group when they
are ready just by adding their name to the group list.  By third grade
though you don't want to move them just because of reading level.  Most of
their levels will be close and you want to have kids who can model good
comprehension skills with kids who need that help.  Also don't want your 2
class clowns in the same group just because they are the same level.....It
is good to start with reading levels and use short books until you see how
the group interacts and learn the needs of your kids.  

Interesting thought though the checklist...3rd grade teachers???  What would
you put on the list?

Fluency, comprehension (what helps them- picture in their head, context
clues, do they see the big picture)  How they decode unknown words...
Book interests-fiction, non-fiction- how do they navigate each (headings or
chapter titles..are they using them to help themselves make predictions)
diagrams/cut away- do they know how to use them I don't know..it would be a
LONG list.  

You will be in quite a different boat from your 1st graders! I taught
Reading Recovery for one year and I have a whole new respect for 1st and 2nd
grade teachers!  I feel like I get the best part when they finally figure
out it is about the STORY, not the words!

I have always taught 1st grade and have had a great system in place  for 
taking notes during guided reading groups.  The forms I have are suited  for

first grade.  It was designed so that I can check off observable  behaviors 
at different text levels along with space for anecdotal notes and a  spot
for 
dates when student moved to the next reading level. 
 
This year, I will be teaching 3rd grade. I am interesting in how  others 
record notes about their individual students while in reading  groups.  I
know 
some people use "sticky notes" but I would not like to  use those.
