Alternate description:

6 foot tall, no head, mouth in chest w/ tentacles ringing it.  Human arms and
legs.  No eyes/ears, "sees/hears" by <telepathy/blindsight/magic senses> 

AD 2:

Starfish-like, upright, uses two tentacles for "arms" and two for "legs" at
will.  Mouth in center.
 
AD 3:

Human, bonded to some extradimensional horror.  Mouth in chest, tentacles
ringing it.  Wears long red robes to cover the mouth, until its time to feed.
