I have always put the cartridge in first, and the few times the film 
didn't load properly, I could see it right away when advancing those 
first couple of blank frames. It has happened rarely, though -- about 
as often as the occasional misload on earlier Pentaxes with 
"traditional" take-up spools.

What I do is insert the leader between some of the needles all the 
way on the right, where the spool has already started curving 
downward, so that inserting the film causes me to start pointing the 
film downward and back toward the supply side. I also always take up 
the tension on the rewind crank, pulling the film flat, and I adjust 
the film to make sure it's lying straight and true, and so that the 
sprocket holes are lined up with the teeth. Perhaps if the teeth 
still need to find the holes and catch them when you wind on, it puts 
enough tension on that take-up spool that the leader can slip out 
quite easily.

This usually works very well for me in all types of cameras, 
especially older ones.
