Actually, I have never thought about that problem, since mutt at least makes an effort to figure out what thread it belongs to. The major problem is users that use an existing message as a template for a new message and their mail client does indeed include the in-reply-to and/or references header. As far as I know, no mail client makes the quoted material read-only, i.e. you can edit it manually. Sure, it is a major PITA to have to do that (especially with Lotus Notes), but you waste everyone's time if you don't do it.
