Before moving on to dbmail, consider the reason why most sites work with simple maildir [courier/qmail]/pseudo-maildir [cyrus] setups: simplicity. The less moving parts, the less can go wrong.
Note that MS Exchange uses an SQL DB for its mail storage, and we all know what a *pain* it is to maintain Exchange, right? There also has been a discussion re this topic on the postfix mailing list[1].
