Although that isn't strictly true.  We do have a way to insert program changes 
in the middle of segments (and by extension, tracks) but it is horrible to 
use, and broken, because it is not possible to send a bank along with the 
program.

I intended to roll up my sleeves and fix this myself, but I gave up quickly 
when I hit a brick wall.  I can't remember the details of the brick wall, but 
it had roots somewhere deep in base/ and had the potential to break things 
all over the place if I altered the fundamental nature of the poorly 
conceived base class, which was designed without any understanding that a 
particular patch occurs at a bank/program address, not just a program 
address.

There was some other problem with trying to pretty up the interface with 
verbose program names too.  No track parameters back then, or maybe it was 
this whole track/segment/instrument relationship we're talking about altering 
now.
