Following up on this confirmation that there is a P1 bug in FM 2015 patch
3.0.3.1 that prevents TOCs from being generated, be aware that it was still
available from Adobe as part of the automatic updates as recently as Monday
(when I got it).

On FM 15, I'm working with a product name that I want in a variable that
contains dashes/hyphens (like Jack-In-The-Box).
The Frame text key sequence of Esc - h doesn't work, and the ASCII sequence
of Alt 2011 doesn't work.
Any ideas?

That would sort of be my point. If you want to -- not have to -- you would
create your gold copy of the graphic at say 300 DPI and downsample it
automatically in Photoshop or simply adjust it to usable size when you
import into FM since you specify the DPI during import.

on a corporate license plan.
FM 12 is faster and more stable than 11, IMHO, so you may want to go there

I'd second John. Current FM output ain't bad, and if you want better,
MIF2Go RTF is really good. I'd stick with FM and upgrade to a non-antique
version (speaking with 4 year's Flare experience) because that conversion
isn't really what Flare does better.
