Yes you can mouse on the mac with VoiceOver alone. By default the VoiceOver cursor does not follow the mouse pointer but you can always route the mouse pointer to the location of the VoiceOver cursor with vo-command-f5 then simulate a click with vo-shift-space. This does not always work in all
