The fact that these messages are coming to stdout causes me to believe
that some supervise daemon associated with Courier in
/var/qmail/supervise is still running and must be stopped.

That is what I would do if you'd rather have Courier, but
maildrop-noauthlib-toaster REQUIRES dovecot as maildrop-toaster requires
courier-imap, courier-pop3, and courier-authlib. So you will have to go

Check you ClamAV logs to see if this is causing the error.
Verify you have the latest version of ClamAV as 0.975-1.4.1

domain info is not store in separate tables, instead that, all domain is
stored at vpopmail tables. Do I miss anything? How do I do to revert: one
table for each domain?

configured to authenticate.
Check to see that MUA (Thunderbird, Outlook, or whatever) is using name
(entire email address, including domain) and password for authentication.
