You could just substitute the source and comment out every patch that conflicts
with it, but it's tedious.
I usually start from rh rpms and just add some patches here and there.

For other examples of kernel rpms, some simpler (but it's seems they build
without the buildroot), see:

you find spec files of caldera, suse, mdk, rhl.

suse is very very short.
