I've been having similar problems, using SA 2.63, called from procmail but that doesn't seem to have anything to do with it.
I haven't been able to find the cause so far, let alone solve it.
At the moment I'm not experiencing the problem, but since I didn't actually change anything I'm sure it'll be back sometime.
