The monitor is called Apple Color Plus 14" Display built Dec 1993
Unfortunately the Formac card has VGA connector and Apple 14" has 
Apple connection.
I have an adapter Apple-to-VGA but not other way
I did try connecting ATI when it was in recently and there was a 
scramble image that was roughly the same color as the desktop but 
completely unreadable
When it was doing its double image trick you could just about read 
the text and the Monitors Control showed only one resolution 1152*870 
@ 75Mhz if I remember...
But that was booting under 9.1 and before the Formac card arrived I 
can't get back to that state now
