#1:  The top bay, which typically holds the CD-ROM.
#2:  The 5.25" bay right below that.
#3:  The 3.5" bay below that.
#4:  The 3.5" bay below that.
#5:  The floppy drive bay.
#6:  The half-cage bay below that, into which a drive can be mounted
circuit-side up.
#7:  The vertical bay behind the speaker, running from the bottom of the
case to the top of the PCI slot region.  The drive is mounted circuit-board
forward, interface & power pointing straight up, with two screws running
through the metal frame behind the front bezel and a flat SCSI ribbon cable
snaking through the gap in the top.

Yes.  See #7, above.  It takes a bit of practice to open it up (basically,
you pry it gently down and back, towards the logic board).

Yes.  The floppy drive is removed by pulling off the front bezel and then
lifting up the two silver tabs below each front corner on the floppy drive.
Pull the tabs forward and out comes the floppy.

Bay #7 is specifically designed to house another fan, though I can't recall
precisely where -- and possibly only if there isn't already a drive in
there.  It's a great location -- cool air is drawn in low and in front, hot
is expelled high and out the back.
