I don't work for a university, but my organization does the following:

For administrative policies - they become effective as soon as they are 
uploaded to our online P&P database, which is usually within a week of them 
being approved by Senior Management.

Our patient care policies are uploaded on the 1st of the month following their 
review and approval at the system P&P committee. Occasionally, I will have 
policies that are published before or after that date - it depends on how big 
the project is, the type of education required, and if anything needs to be 
changed in the electronic medical record first.
