Personally, I think the "no refunds under any circumstances" policy is
a good one for small amounts like users pay for apps. It shifts
control of refunds to customer service and there is no obligation to
offer anything to anybody. Any other policy would invite abuse by users.

This was actually the first time in 4 years that anyone actually sent a
refund request. Most people do so in the 15 minute Google window. A few
others have emailed me and said they wanted a refund but also because of
something they didn't understand about the app. After I clarified what
they were doing wrong they were happy and some responded with a comment
with 4 or 5 stars.
And if you go to the developer Distribute page and scroll to the bottom
and click on "Issuing Refunds" link you get "Page Not Available."

I agree that having any other refund policy is opening yourself to abuse.
