If you look at the request in the Service Requests form, on the assignment tab is the co-ordinator field blank? It should have the co-ordinator group that you specified in the SRD. Also when you submitted the SR, did it create the fulfillment records (Work orders/incident/change) or did it error out? (Check the process section when viewing the submitted request via the details button from the request entry console)
