Bernie,

A 'w' would add a .5 second pause in the dialstring.  So, where you're
specifying the 8 for outside access, try using ww8ww.  It's unlikely
that this is the problem but it'll go offhook, wait to allow the CO to
give you dialtone then dial 8, wait to allow the dialtone to come back
then dial the rest of the digits.

I assume you've plugged a regular phone directly into each line and
dialed to ensure that the lines have been provisioned properly by your
centrex provider.

When you say you can dial 411 on line 2, is that through asterisk?  Do
you have to dial 8411 or just 411 if you've got a phone directly
connected?

Maybe you should try putting line 2 into group 1 and leaving line 1 in
group 0.  Then you can deal with one line at a time.  If you swapped
the lines, then it certainly sounds like the problem is related to the
Asterisk port, rather than the centrex line but try this stuff and
let's see where we end up.

I remove the prefix 8 from the box and code a dial string 8+NXXNXXXXXX
in the dial rules it dial with the exact same result as above. Is there a
pause I can add to the dial string to slow down the rule

When I originally built the box I had only 1FXO in the box and upgraded to
the Digium TDM400P after, this couldn't be a problem could it??
