In the trunk menu, try specifying the zap group as ZAP/G0 instead of
ZAP/g0. The capital should use a descending trunk selection order.
That will confirm your observation that the problem is the second
channel to be selected, not the physical 'channel 2'.

BTW, I'm also pretty puzzled.

When you watch the call go through from the CLI, and it's dialing the
number, does it show the 8 in front of the number?

Are you using 9 to access any other trunk group on your asterisk box?
IE What would happen if I were to dial 9-416-555-1212 on one of your
phones?

Looks good so of I go to my phone tie up channel 1
put a call out on channel 2 and It goes through not great sound quality but
it goes, I think problem solved, not so

I realized I plugged a live phone line into channel 2 but not 1 I forgot to
hook it in yadda yadda.
So I hook up line 1 make a call no problem can't get line 2 to  work
again.  I move the line to channel 3 from channel 1 channel 2 calls out but
3 is fast busy. It seems that if there are no calls established it will
work on any channel but one channel down it does not issue the prefix for
my centrex.
