Thanks, but I am not "modifying" per correction policy, but actually inactivating. In 
reality I am modifying, why do they make it so complicated?

You are modifying the document to reflect the error made. You do not have to change 
the R2b, the date the RN signs that the assessment was completed. When you sign the 
attestation the date you make the modification is what you us. I usually put in those 
lines on page 1 of the new MDS "Correction make to section ___ and sign it. You do not 
need to get the team to resign because their assessment did not change. If you change 
the R2b your assessment will be rejected.
