Thank you, I think I understand what you are saying
now. I guess the only thing I have ever inactivated was a discharge
tracking form and R2b is not on the discharge forms. I appreciate you
explaining that.

You are trying to apply the rules for
modifications to this situation - inactivation - which has different rules.
The "new, original MDS record" that you are submitting was not "completed on
the original R2b" date. It was completed after all of the data was input
into the record. That would be the new
R2b.

Even if all the information in the assessment was the
same and the only thing that would be different is the type of
assessment? I guess this confuses me because to me the assessment was
completed on the original R2b and to me the assessment is the same.
Why couldn't you date on the attestation form the date you inactivated the
assessment? To me the assessment of the Resident did not change and
they completed the assessment by the original R2b date. Fortunately I
have not had to do this and hope I don't have to but if I ever do I just

After inactivating the record, the next step is to create
and submit a new, original MDS record. Since this is a new, original
record, the R2b date would be the date the new assessment was completed -
not the date the inactivated assessment was completed. See the MDS

Okay, I have inactivated the 60 day
assessment that should have been a 30 day....I have redone the 30day and
am getting ready to resubmit....what do I sign as R2b?????
Help, this is driving me crazy, I have never had to inactivate a record
and I am freaking........I want to keep the same date on the R2b and
from reading the Correction policy, I think this is correct. Any answers
are appreciated.
