The initial email address provided by Cynthia was not a registration email
address. This is why the system could not recognize it. The system cannot
send registration codes to unknown emails, as you understand. After
additional request I had to manually search for the real registration email
address by additional information provided. Once the email was found it has
been sent to Cynthia so that it is used in the automatic system. However
Cynthia reported that she could not get the Activation Code to this new
address also. So I sent the activation code manually one more time. In both
cases the message was placed to the spam folder. The issue has been
resolved finally.

So your request was processed by support? If you did not receive the email
please send your registration email on support one more time, I will check
your registrations and will send the upgrade coupon manually to other email
address if needed. For now I cannot recognize you from this account in our
system and support activity.

Yes, this is the reason in most cases.

My guess is that the issue is in the nature of the Activation Code itself,
which is not easy to change.
