Thanks for the tip. The ACMI list a good starting point, though the list is
not complete. It also appears to be an inclusive list, it lists hospitals
that have midwifery run units that aren't necessarily 'Birth Centres'.

options open to your friend include seeing either a physiotherapist, osteopath 
or chiropractor. Ideally see a professional that has a special interest in 
pregnancy. All use different techniques with varying degrees of 
success.

There 
are special pregnancy exercises that can help elevate backache in pregnancy - 
your friend would need to be shown how do do these exercises. A physiotherapist 
or some midwives will be able to show your friend these 
exercises.

There 
is a website in the UK on Group B strep - it has leaflets as.pdf files 
that you are able to download - Check it out at http://www.gbss.org.uk/. I 
providewomen with one of the leaflets off this site and talk about the 
variations in policies at different hospitals. Some women will choose to have a 
swab in pregnancy (generally late pregnancy) - if positive I suggest a 
naturopathic/homeopathic consultation. I discuss IV antibiotics in labour at 
home and their feelings around the same. On a couple of occasions women have had 
IV antibiotics at home - heavy growth of GBS remained after they had taken 
natural remedies. If a woman has SROM and no labour - a recommendation is made 
to have a low vaginal swab as soon as practical (given it takes 48 hours for the 
final result) - so her GBS status is known.

I too have been faced with the no bath policy at RPA - except there was a
plug in the room - so the woman filled the bath regardless of their policy.

I faced an even more serious problem at the same hospital. A woman I was
caring for was booked to give birth at RPA birth centre but had heavily
stained meconium liquor - so following policy we rang the labour ward and
was told that they were full and we couldn't come in. We were redirected to
Canterbury Hospital. When we arrived at Canterbury Hospital we were told
this was the forth time this has happened and the new unit had only been
open for 2 weeks at the time.

Remember the hoo-ha when the plugs were removed from the baths in The Royal
Womens Hospital in Brisbane! Well, the silly policy season has struck again
- I have just been to a birth at the new RPA Womens and Babies Hospital
(was King George V) and in their labour ward they have lovely big baths but
no-one is allowed to use them - it is not policy and to make sure, there
are no plugs!!

a plug with you and be prepared for a battle!

When we set up the company we were insured - 
however, as you can guess, this is no longer the case. We are still continuing 
to submit applications to various insurance companies (as time permits) with 
risk management procedures and proposing to incorporate an excess to make us 
look more insurable - so far no luck - but we haven't given up trying. I will 
let everyone know if we are ever successful.

Dear Rhonda

I work in a practice of 4 midwives and we 
decided to incorporate due to the legal advice we received. We are now known as 
Sydney Midwifery Practice Pty Ltd. 

It has been a big process and we are still 
learning. There are many rules once you become incorporated - from BAS, to 
taxation issues and super. We are still learning and am finding it time 
consuming. But it is unfortunately necessary.

Our solicitor, in light of our situation, and not being able to get 
insurance has suggested that we become Incorporated. 
Because they have to sue us personally as the person who performed 
the procedure - all of our assets would then be owned by the company and 
could not be touched by personal litigation. 
I guess this is something we never thought we would need to do but 
I was thinking that midwives who are working without insurance could do 
the same thing and therefore protect themselves at least from personal 
litigation.

I know it does not solve the problem - a bit like the "form to fill 
out" it is just another measure to protect oneself.

Has anyone else considered this option?

The normal range of Ferritin in 10 to 150 - so 14 is in normal range. A lot
of my clients have ferritin levels taken - and most of the time they are
around 10 to 20 in the last trimester of pregnancy.

Hb levels above 105 are also normal

Needing some help to clarify the difference between haemoglobin levels and
ferritin levels.

Have a local GP who switches between the two readings depending on which one
is lowest and suggests/insists on iron injections.

Levels I've had quoted from some of the women are:

Hb  107
Ferritin   14


another: Hb 109
Ferritin 13

These two women are both 32 weeks.

Just needing clarification and some evidence about the relevance of
both/either readings. Hb levels seem fine to me - a bit foxed by the
ferritin level - one woman had dropped from 120 early pregnancy to 14 now...

A book that always stuck in my mind was called 'Medical Dominance' - the
author was Willis (sorry I can't find a full reference). There was a chapter
on midwifery and the process of medical dominance from settlement in
Australia. The book is out of print - but you should be able to get a copy
in a library.

'International Journal of Childbirth Education' (IJCE) the official
quarterly publication of the International Childbirth Education Association.
As a member of the group s*ubscription the cost is $54.00 Australian - It is

Inaugural meeting for Maternity Coalition - NSW Branch

Exciting news - Maternity Coalition NSW Branch is about to be born

A lot of work is going on behind the scenes preparing for this momentous
event. Maternity Coalition, as most of you know, is a national group working
for positive changes in maternity services. Birthing service consumers,
mothers, fathers, midwives childbirth educators, activists and any other
interested parties please join with us to help implement change.

After the election of office bearers, the number one item on the agenda will
be putting together a plan for securing Professional Indemnity insurance to
help ensure that the option of having a midwife in private practice care for
women through pregnancy, birth and beyond remains a viable possibility.
Private midwifery care is on the verge of ceasing to exist.

Other items on the agenda (so far) include: The Australian Midwifery
Campaign (to achieve for all women the right to choose a midwife as their
primary caregiver during pregnancy and birth within the health system
[public and private] whether in the community or hospital) and establishing
‘Choices for Childbirth’ evenings.

The inaugural meeting is planned for the 21st February 2002. Venue is: 27
Hart Street Dundas NSW 2117. Time 7.30pm

An agenda will be emailed to members and interested parties prior to the

the best references I have found on exercise in pregnancy is a book by Dr James 
Clapp called 'Exercising through your pregnancy'. James isone of the 
worlds foremost researches in the area of exercise and pregnancy. I have a huge 
collection of research articles and books on exercise and pregnancy and found 
this book to be the most helpful. It also explodes some of the myths about 
exercising during pregnancy and looks at lots of research in this area. From 
memory I had to purchase it through Amazon.com - as its published in the US.

Independent midwifery is alive and well in Sydney. There is only around 10
midwives in Sydney currently practicing. A number of midwives have stopped
practicing due to the withdrawal of professional indemnity insurance. Have
emailed Leilah directly with the information she requested. The Australian
Society of Independent Midwives is active in Sydney as is Homebirth Access
Sydney (homebirth support group).

like to differentiate between copyright infringement that Joy and Pinky are 
talking about and stories for the 'The Birth Book'. In no way did I infringe 
copyright.

People 
had emailed me privately with their birth stories after a request for birth 
storiesfor inclusioninto the'The Birth Book' was placed on 
ozmidwifery. A follow up email was sent to all those who sent me stories 
confirming permission. Though I do think Pinky's written permission is 
agood idea (which is something I now do) and so is having better back up 
for the computer (which I might add I now have in place).

Pinky 
-I can request a copy of the book from the publisher for those who had 
their stories reproduced. I'm sure the publisherwouldn't 
mind.

  I have also come across sections from 
  articles I have written in various newspapers that I was unaware had been used 
  in Books - I dont mind if I am acknowleged and people have toldme 
  EXACTLY what they are using but I was shocked to find a direct quote 
  ina book on body imageby a well known Melbourne writer/ cartoonist 
  a few years ago that simply referenced my article as "Belly Dancing 
  article Sunday Age ". -Not even my name as the author and certainly no request 
  for permission.

  had permission forms filled in by people I interviewed - even thougn I wrote

  who supplied their own stories were offered "payment' - most requested 
  copies of the book.

The Birth Book was released at the end of last month. I had the pleasure of
being involved in its making.

I requested birth stories on this list to put in 'The Birth Book' about 18
months ago. At the time I was sent around 5 stories with permission to
include them in the book. The stories were forwarded onto the publisher -
who chose the stories to include. Unfortunately when I migrated information
onto my new computer earlier this year some data was destroyed - including
the contact details of those who sent me the stories. So if you sent me a
story - let me know and I will let you know if your story was included in
the book.

I was absolutely delighted yesterday to completely by accident discover that
my birth story was published in The Birth Book, Carol Barbar (revised
edition published this year).  I was in a bookshop with husband (who is the
IT Manager for this chain of bookshops) and was browsing the preggo/baby
titles (as is my habit), and this one was face out (bookselling term) and
I'd never seen it before, so thumbed through, and saw Jo's Birth Story.
The first sentence was so familiar!  And then it dawned on me.

I am pleased as punch, and not at all seeking any financial or other reward,
but was just wondering exactly how it got there.  I think that I probably
copied it to a request from this list for birth stories (and never thought
anything of it) and, as I say, I am not after anything, but was just a
little surprised, as had I not discovered this, I may never have known it
was in print for real!

So thank you, and if anyone can put me on the trail of the responsible
(bless them), I'd like to say thanks properly.

Fridays PI insurance meeting in Sydney

I have a previous client whose husband is the head of government relations
for Telstra. He has offered his services free of charge (usually 100's of
thousands!) for Fridays meeting. Lobbying the government is what he does!!
What he needs us to do is find evidence that homebirth is economically
viable, and that it is cheaper to birth @ home than in hospital from the
governments point of view. I know this info is out there but not sure where
to find it.any suggestions or does any one already have it?

Blackmores make a morning sickness formula that contains ginger and Vit B6 -
many women find it provides at least some relief. You cann't take Vit B6 for
prolonged periods of time though.

Many thanks for forwarding 
information regarding your attempts to arrange continuing indemnity cover for 
independent midwifery. I think that it is unlikely that a private insurance 
company will be prepared to offer the type of cover that you need at a price 
that you can afford. Therefore I think that you should push for inclusion in the 
Treasury Managed Fund(or equivalent in other states)which covers all 
nursing and medical staff in public hospitals. The fact that NSW Health 
acknowledges that homebirth should be an option places some responsibility on 
them to help you in the present crisis. Health Departments would be more 
receptive to the proposal if your Association agreed to participate in peer 
review etc.

I note that one of the options that 
your colleagues are considering is "going bare". A signed statement from your 
clients to the effect that they were aware that you were not indemnified would 
probably have no validity in the event of a claim. In the present medicolegal 
environment the only avenue for financial assistanceopen to parents of a 
child with major neurological abnormality is to lodge a claim against the birth 
attendant. One could argue that it is ethically improper to deny that access to 
assistance by deliberately working without liability cover. The Health Care 
Liability Bill presently before Parliament in NSW will require doctors to carry 
indemnity cover as a condition of registration and I expect that this 
requirement will flow on to other health professionals.

Dear Katrina

The insurance provided by ANF is only provided by the Victorian branch. I
tried to pursue this yesterday - but because I live in NSW - I cannot become
a member of ANF Victoria (something to do with their constitution).

I spoke with David Marshall - ANF Victoria's insurance broker yesterday. He
claims that no company in Australia will cover midwives. He is currently
trying to find insurance off shore for midwives (in the UK I think) - but it
will come down to cost (the exchange rate between here at the UK isn't
exactly great).

I contracted my own insurance broker who put in submissions for me to all
companies who do professional indemnity insurance in Australia - there are 8
companies and 8 declined to insure me.

I also contacted the NSW Nurses Association yesterday - I know we have
philosophical differences - however they have political clout and huge
membership. They contacted me back and asked to write to the general
secretary - they may well take it up as an issue. The person I spoke to
quite outraged at the current state of affairs.

As a hospital-based midwife and a consumer of private midwifery services in
the past, I have been interested in the discussions regarding insurance for
midwives in private practice.
One point is unclear to me...I am wondering what the problem is with
insurance provided through ANF?  Is it a philosophical concern about
midwives being members of a nursing organisation, or are there other issues
regarding this insurance?  If so, what would they be?
I read that some (Joy, I think)have only insurance through ANF, and wonder
why these midwives consider it to be suitable and others don't.

Another thought that comes to mind is does anyone know what ANF's position
on Batchelor of Midwifery graduates will be? I know that ANF is not in
favour of direct entry midwifery, but will B.Mid midwives be accepted for
membership of ANF, not being nurses? I would imagine that some B.Mid grads
will end up working in hospitals or other health care services, and wonder
where they will get industrial representation?
Is this something that ACMI is ever likely to take on, so that we can all
drop our membership of a nurses organisation?
The problems faced by midwives in private practice is an issue that all
midwives should be concerned about, but many hospital midwives don't see it
as affecting them in any way. I get really frustrated by the midwives where
I work who spout off all the time about how much better off our clients
would be with a midwives clinic, and how we should be able to have
midwife-managed care for selected women, but then when they become pregnant
themselves, its straight off to the private obs/private hospital package.
If we can't support ourselves and present a unified campaign, we're never
going to achieve government and community recognition for midwives as care
providers.
But I guess I'm preaching to the converted here!
