Unfortunately, my card (as well as most others, apparently) didn't have the jumper pins installed at the factory. So, being kind of a hardware hacker, I decided to add the pins, install a jumper, and see what happens.
Viola!, now I have no more issues with sync-on-green, and I am able to use this old Twin Turbo in OS X without any issues.
Soldering a 3-pin jumper block onto the card wasn't that difficult, and has given this card some additional life for now.
I have an 8MB TT sitting on a shelf, because my old portrait monitor doesn't support sync on green. You don't happen to have a photo/scan of your soldering job do you?
btw, I also have a 2MB and 4MB cards which have the jumper, but the 8MB doesn't. Is it in the same physical location on the cards?
