I do the same thing.  I call it the "pain-in-the-ass" factor.  Things like
"I need to get out of my chair and into my car to come to a meeting"
and "I need to provide daily status reports" and "the specifications
have changed" rate high on the pain-in-the-ass scale, and I charge
accordingly.

And I'm still continually astonished that clients will actually pay
these charges instead of getting their act together and avoiding the
whole ordeal.  So perhaps a more PC name for it would be "disorganization
surcharge". ;-)
