at a guess, someone has a link to your site bookmarked on a local server.
the fact that the schema section of the URL is 'http:/' indicates that the
page is being served by an actual websrver. the fact that the machine
name is only '/www' says that the server in question is in the same local
network as the client which uses it. the '.htm' on the page name says
that it's probably a Windows machine, so it's not unreasonable to speculate
that someone is running ISS in the background, and calling up pages from
their own hard drive through that.
the '?plugin' bit doesn't make much sense to me, because that extension to
the URL is only supposed to contain arguments for a script. i *suppose*
that if the URL in question was redirected to a script, that argument would
be meaningful. OTOH, there may be quirks in the way MS has chosen to
interpret the structure of a URL which "make things better".
a local page of some sort, with some kind of script involvement, is the
general range, though.
