Me, I've opted for Choice #2, but as has been pointed out, users need to understand that something has happened when they click 'Add to Cart'.  Originally I had the box pop up in what I thought was an obvious place.  However I heard the occasional report of a user who was unable to figure out what was going on.

So I changed from a nice box that blends harmoniously with the design and is one nature to a nice box in a contrasting background color (i.e. if site is dk blue then box is white) with a box inside of that colored screaming bright red that shows the cart item and amount totals in either white or pale yellow text.

Also, the box is always present on the upper left corner and, when its empty it says "your cart is empty".  I think this makes the change more apparent when it happens as the person at some point typically notices the prominent box prior to beginning the cart addition process, and thus has a cognitive leg up on the situation when the cart contents change.

All of this is guesswork on my part of course, but the net result has been zero problems since I went to this method.
