Re: [QUAD-L] Blood Clots - Picc line vs port

2009-10-23 Thread Bill Jenkins
Discuss with your husband and doctors having a Medi-port put in his chest.
I've had one since 1998 and it's great. Less risk of clots, easier access for
IV's and not hard to flush and heplock.

I need help deciding what to do for my husband. He is a C-4 complete with a
trach and a peg tube. He was hospitlized for a year and has been home since May.

Since he has been home he has been hospitizied 3 times with pnemonia and is
currenltly in the hospital. When he goes to the hospital he needs IV meds.
Currenltly his IV is in his foot because his veins are hard to get at. The
hospital wants to put a pic line in again. I have told him no at this time
because his vein is still working. When he was hospitlized (first year), he
got a blood clot in his arm due to the pic line. I am trying to prevent that
from happening again.

It has been suggested from our home health care agency to get a port
installed, so we can do IV's at home. The doctors have told me they cause
blood clots as well.

I am trying to make the best decision for my husband without causing anymore
complications, especially blood clots.

Can anyone tell me what is the lesser of the two evils? Picc line or Port?

  Please provide me some feedback.

FYI - My husband got MRSA when he was hospitalied initially. It comes and it
goes. I was just reading that the regular IV's can cause that as well.\

  I don't what to do.

  Please help.

  Thank you,

Kandy - wife of C4 complete, accident 5-8-08. ( My husband has many

In 1998 '99 I was having a lot of trouble with my legs. I ended up having
both amputated in 2000 due to Osteomyelitis, Osteoporosis, muscle atrophy and
cellulitis. I was also having frequent UTI's and had no veins left for IV
access. The medi-port was a great solution for me. It has to be flushed and
heplocked once per month to keep it free from clots.

Tomorrow will be 19 years, just where did the time go? I am now 70 ½-that
can't be true either. JIf I had a choice would I do it over again? Most
emphatically yes. I have three grandchildren I would not have seen otherwise,
one great granddaughter, a great granddaughter due in December, and a
granddaughter due in January.

What more can a grandma ask for?

I am not looking forward to some of the things that lie in the
future-probably more skin problems because of thinning skin-worse hearing that
I have now-right now my Some Timers Disease is OK, but maybe it will get worse
and I suppose I could find a bunch more.

I Am looking forward to some of the things that live in the future-new
grandchildren and great grandchildren, watching the others grow up and tackle
life as best they can, finishing my garden (probably not), enjoying the
greenhouse and fresh tomatoes in winter, staying involved in youth activities
(I love to see how children get comfortable with me in the wheelchair and only
think of me as a lady who uses a wheelchair to get around rather than a
disabled person), maybe writing a book-don't know who would read it but I might
enjoy writing it-, helping friends and family landscape their houses-I get the
fun part, designing it, they get the fun part of doing it-continue taking a
couple of college courses that interest me. OK, I guess I get it. My list as
much longer for things I want to do than it is for things I'm afraid of. So I
will continue on my merry way-probably not for another 19 years although my
mother lived until 91 after suffering a major stroke 19 years before, so who
knows what is my lifeline to be.

Glad I have this list to belong to-it is good to share with others in the
same boat.

I'm finding that as time passes, many things are getting harder for me to do.
I used to stand to transfer and used a manual chair for the 1st 38 years of
this 'quad' thing. Now I'm electric - and my fingers and shoulders feel much
better but getting dressed is much harder now as is getting into bed. I think
I know where this all is going and I don't like it at all. I made a bad
transfer a few years ago and heard something down under 'crunch'. As it turns
out, it was my urethra which then leaked into my perineum and within days I
had a uti that nearly killed me. I now take all my transfers very carefully
and slowly making sure I don't goof up. I come down my van ramp backwards so
I don't kiss the driveway - again, and I always have my cell phone with me just
in case. In our cases, an ounce of prevention is definitely worth a ton of
cure!! Be careful out there.

Hi Billy. Bill of Il.here. I share your sentiments and emotions. I too,
lived my life as you do, dead from the neck on down, but my brain never quit
working. Unfortunately, as you say, some insinuate that we must be as equality
retarded as we are handicapped.

I understand that blind people often experience something similar to this
same event. Seems that when individuals learned the person was blind, they
immediately began talking much louder and had to be told, Hey, this person is
blind, not deaf.

Example. At one point, when I was 60 years old, I was told that I had only
24 hours to live, and that there was no hope for me because the tumor had
crushed my spinal cord to the thickness of a ribbon. Those were the words of
my neurosurgeon, a great and wonderful doctor.

After surviving the removal of the tumor, I was told that I was lucky to be
alive, but that I would live as a vegetable for the remaining time that I
remained alive. I spent months lying in bed, not being able to turn over or to
even feed myself.

I remember lying in bed, doing the only thing I could do on my own, and that
was to watch life go by. When I tired of that, I would yell and someone would
turn me away from the window. The longer I lay there, the more determined I
became to make an effort to change my situation, if only in the slightest way.

So slowly I began to fight back against this thing that tries to destroy us.
I remember the first time I tried to sit up. Someone had to hold me in a
sitting position or else I would fall off the bed. It took several months, but
I finally reached the point where I was able to sit up unaided. What a great
feeling it was to experience that feeling of freedom.

We have all been robbed of our lives and dreams and aspirations by an enemy
we never knew. We didn't ask for this life, so why should I give this enemy
every last once of victory. Every time I learn to do something that was taken
from me, I win back an once of freedom. I take back what has been taken from
me. Like Billy stated, I am not brain dead, and I have an awful lot of time to
think as long as I am sitting there, so I began to outsmart the enemy and
figure out ways to succeed in the simple tasks.

One day after a lot of work I actually buttoned one button on my shirt. Yes,
in a matter of seconds my PC person could have done it, but that person (as
wonderful as they are) will never experience the sweet feeling of that success.

I never, never belittled anyone for trying to help us. These were
embellishment written by someone else writing on the list. The analogy was
given about an elderly lady crossing a street. Should we sit idly by and watch
her struggle, or should we run up to her and announce that we are going to help
her cross the street. We do neither.

We first approach and ask her or him if they would like any help. This
satisfy's the human urge to help those in need, but remember, we can only
respond to the elderly lady's response. If she says why yes, thank you, we
then her. but if she says, no thank you, I want to attempt this crossing on
my own, we can only capitulate and watch her cross the street. No one is
demeaning those who are care givers, but if any of you were to come up to me
and ask me if you could do assist me with somethingBill o I was trying to do, I
would simply tell you not at the moment, thank you, I want to fail first.

Today, as I write, after 6 years of trying, I can walk a little bit on my
own, even though I was told that I would never be able to walk unassisted. I
am not through declaring war on the enemy, and will continue to do so until the
last hour before I die, which time is always near. When I do check out, I plan
to do so with a smile on my face, because I gave that enemy(whoever that might
be) the best fight I was able to bring to the battlefield, given the condition
I am in.

We had a UHF station in Detroit (ch. 50) that we were able to receive starting
about 1961. There was a guy named Bill Kennedy that hosted a show that played
nothing but classic movies. Kennedy's claim to fame was playing the
executioner in the film The Hunchback of Notre Dame. He had only 1 line in
the movie where he was lighting the fire to burn Qusimoto's love at the stake.
It was Hand me another faggot. A faggots was a bundle of sticks tied
together. Not a derogatory term for a gay person.

As you can see, my answer is Yes. I love classic films. I watch TCM and AMC
quite often.

  Good Morning:

Being a great fan of the theater, I especially love the old black and white
films of the 20's, 30's 40's and 50's. I guess the reason for this is that I
grew up in the era and genre.

So it was with great pleasure that I found a good Bogart movie last night
that I had never seen before. It was entitled,'Marked Women' co-staring Betty
Davis and was a 1937 production. As Bogart starred in mostly B movies in his
early career, it wasn't a bad production. It was also quite evident that
Bogart had not reached his potential yet.

I am curious to know if by chance there are any other fans of classic moves
here. Those of us who share the love of the black and white cinemas talk by
the hours about the likes and dislikes of the classics and sharing bits of
trivia. For example, the Bogart movie 'Casablanca has been rated as the most
Romanic movie in the history of US films.

Another circa favorite of the black white films are the Bowery Boys,
produced from the late 30's up through the 1950s. These films have developed
a cult following and produced such loveable characters as Slip, Sach, Chuck,
Whity and Louie, the sweet shop owner. Anyone on the list old enough to
remember these films? Some of these films were entitles 'Hell's Kitchen,
Angles With Dirty Faces and The East Side Kids'. Much thanks goes to Ted Turner
for airing the TCM television network which airs these classics 24/7 commercial
free.

Perhaps I might just invoke interest in this subject or even the associated
field of literature, but then on the other hand this subject might be perceived
as the ranting of an old man.

help, I'm always happy when someone opens a door or picks up something I
dropped, etc., etc. It makes my day to know that in this increasingly selfish
society, there are still people who will take a moment to help someone out.

My type of disability won't get better - only worse over time. I
remember having a discussion with an OT and she said 'Sure Dan, you can
struggle to get your shirt on or cut up your food etc. but how long will these
things take and at what expense to your time and energy reserve? If a PCA does
these things for you, you can save time and energy for the really important
things in life - like masturbation or working on the computer'.

And Bill, would you say the same thing about watching an elderly lady
with a bad limp and using a cane struggling to cross a parking lot carrying a
large bag of groceries? Go granny, go! Come on you can do it and if you have
another stroke trying at least you gave it your best shot. Bravo!

Dan - who is grateful for the compassionate ones.

I quote from one of my statements in my last post, Once again, I am only
expressing my own thoughts and ideas on these matters. Please do not assume me
to be dictatorial or presumptuous with my statements.

Or perhaps I am mistaken. I was under the impression that a member of this
list still retained the freedom of expression.
I quote from another source. In a move entitled 'Uncommon Valor', a movie
that is dear to my heart because I served with many of my beloved brothers in
Vietnam. A statement from this movie was made by an actor named Sailor, who
said, Man,you don't ever quit.

Yes, yes, I've heard it all before, on how men like me are politically
incorrect for being involved in that crazy Asian war. I personally don't care,
even though the cause of my t-1 2 condition is from Agent Orange. All I'm
describing is what is best for me. I haven't and I never will quit trying. If
I drop something, and it takes me a half an hour to pick it up, who's going to
complain, or will the politically incorrect police come and arrest me? You say
that it's important to conserve your strength for the important things, well
struggling to exist everyday is my form of freedom, and that to me is the most
important thing in life.

Perhaps the prejudice on this list run the other way, and apparently I
don't qualify as being a legitimate quadriplegic, at least not on this list. I
made a mistake by joining, but that's ok, I make many mistakes. I'm sure there
may exist other lists such as this one, or maybe there isn't. Either way it
doesn't make any difference.

Alan,
How long (if ever) did you push a manual chair? I have arthritis in my
shoulders from over 17 years in a manual. I developed bone spurs in one
shoulder and had rotator cuff surgery. The shooulder problems caused chest
muscle pain because I used them to compensate for weaker shoulder muscles. I
don't know if that's your problem, but think about it.

Thankfully, for some reason, my upper chest and arms are better today
(less pain, heaviness, numbness, and weakness, and I don't feel so weak
overall. Right arm still has some numbness and weakness, but that's been
standard for a while, and it's not nearly as bad as the past two days.)
Yesterday was really scary - the arms were extremely painful, and I could
barely move them, and it got more difficult as the day went on. I really
feared I'd wake up today with no arm use at all. I went to bed at 7:00,
because I couldn't really type any more, and could barely control my chair
  - unless I'm sick or the catheter blows, I never go to bed before I go in
for the night at 10:00, no matter how bad pains are. To explain the
difference between yesterday and normal, when I'm in bed, I do movements
of my arms in sets of 20 (that's sets, plural, for some moves, like
rotator cuff.) Yesterday, I tried to do them after I lay down. By the
fifth move, I had to stop - I couldn't do them. Today, I got full sets
done, and into second sets, and I'm pretty much having only the usual
difficulties using my arms - upper back pain, poor balance and posture,
  and popping scapulas.

What could cause this? Deficiency in some vitamin or mineral involved in
nerve conduction or muscle movement? The inner arm pains definitely felt
like the central pain - the skin and muscles pulled when I moved, and all
the sensations felt like the rest of my body. They came on so fast! I'm
not taking any new meds, and I am eating (not much, but that's normal for
me, and I have my Boost Plus and multivitamin daily.) I don't think
effects of a syrinx would come on that fast, and I doubt they'd get better
by themselves. Anyhoo, I'm much better physically and emotionally today.

  Hey...

For those of you who post regularly (as well as those who do not or just
lurk) I was wondering if you all could tell the rest of us how long you have
been on this list?

I found it in the latter part of 1995 and never unsubscribed except for six
months when we were living in temporary quarters at one time around 2000 so it
has been 12 1/2 years for me or so. During the last 12 1/2 years I have told
at least four people about this list because they had not heard of it. They
know who they are. :-)

Anyway, it would be refreshing to see how long y'll all have been on the list.
Happy new year to you all!

come from the low end products outperforming expectations. The space shuttle

the quickies were a low cost quality product. Anyone else got any feedback on

I was wondering how you folks that qualify for Medicaid and/or SSI (whereby
you can only have so much money in your checking account, assets... cash or
otherwise, etc. etc. etc.) are able to afford some of the better things in
life?

For example, when GHenry recently asked about wireless microphones... this is
just one example out of thousands and thousands of things that are pricey.

Do you just go without or do family members help you with such purchases? Or
really nice friends! Say, for example, that you wanted a brand-new Sony Bravia
flat screen TV that is 46 inches. Or a brand-new MP3 player that is worth it
($200+) and a myriad of other examples.

  Just curious!

Some of the worst effects are decreased circulation in your extremities.
Nicotine and other agents cause narrowing of the blood vessels which causes
poor circulation. This can result in poor healing capabilities in the lower
legs should you get a pressure sore there. I smoked until 1989 and the docs

I just passed my driver's test with flying colors! Now it's time to start
van shopping. I'm
thinking about getting a Chevy Uplander and I'll be driving from the drivers
seat as opposed to my wheelchair.

Where can I go on the net that can explain all the conversion options for
handicapped vans?
(i.e. hand controls, blinker/headlights/horn/etc. options, seating options,
etc.)

Have y'all ever heard of a minivan that had a rear seat that would
electronically lay down?
I'd like to have that in case I was out somewhere and I got tired (medicine
head).

2007-10-27 Thread Bill Jenkins
I hate to contradict you, Wheel, but any licensed physician can write the
prescription if the OT or PT writes the letter of evaluation and medical
necessity well enough. I got my letter from an OT in Feb. '07, my prescription
from my doctor's PA in Mar. '07 and my chair was delivered April 13, '07. IMO
the DME provider makes all the difference by double-checking everything; making
sure every i is dotted, every t is crossed and everything is coded
properly. I chose the Quantum 600 and it's been great. I liked the center
wheel drive because both my legs are anputated above-the-knee making my
weight/balance cause a rear-wheel drive tip easily. It also had the best
options for me: Roho high-profile Quatro cushion, tilt, recline, etc.

You will learn in time that obtaining a new chair these days is a lot more
difficult today than it was just 5 years ago. Especially if CMS is paying for
that chair. First, is the order from a licensed doctor in rehab medicine, to
see an Occupational Therapist for a complete evaluation based on medical needs.
 The
evaluation goes back to the doctor for another order to the DME. The DME
will then determine what is best for you, with your help and assistance and
submit those findings to Gov Insurance to determine if they will be paid for
the perfect chair. Sometimes they get a yes and other times they get a no.
Submittals have to be adjusted and justifications have to be properly coded and
submitted.

The process could take 6 months to a year. Maybe longer. Sometimes shorter.
Ask around and evaluate each answer, based on its user and needs.

  Best Wishes

old with my butt in it almost everyday between 8 to 14 hours. A few signs of
wear are becoming evident. I figure I'll get my new back replacement and
batteries then get them started on a replacement. Unless Ohio has dictated the
chairs shall last 7 years!

My neighbor did put some cool leg armor on my chair so I can visit offices
where they don't allow wheelchairs. (Like Medicaid)

Is that the total choice open to medicare and medicaid patience?

reducing the risk of testicular cancers. (however professional body builders
using this technique in conjunction with steroid therapy for the purpose of

You are a pretty good detective Lori, I salute your efforts and successes.
I had about 6 utis last year, 3 of them requiring hospitalization and fancy new
(to me) drugs Zosyn and Vancomyosin , and finally in November I told the doc to
remove what I thought was the offending part. I had been using a condom
catheter since 1967 and figured it was doing me just fine even tho it allowed
me to retain about 300 cc in my bladder. I was sure foleys were a much bigger
possibility for introducing utis into my system. Now since my orchiectomy and
the use of a foley (a bunch of them actually) since 11-2007, I have had only 1
or 2 minor utis. I now seem to be noticing cleaner output since using collidal
silver, alka seltzer, and vinegar some great home remedies for Bladder
Infections .

Hi Danny,

You and I have been talking (around 2 years now?) about the chronic UTIs
that we both have
been plagued with. But it seems like you have not had very good luck with
competent doctors.
Neither have I but I finally found out that the culprit were those very
big, big bladder stones
that had been sitting in growing in my bladder for years and kept growing.
Anyway, you said that you have had a bad UTI for a month now! If it is
bad then I must assume
it has gone systemic and you have been running a fever with severe chills
for a whole month?
Something does not sound right. Not to mention being extremely
dangerous!!! In other words...

what the F***? have you or the doctors than waiting on/for? After a whole
month... you just
got urine culture results today which only take three days (on the outside)?

Just before, and after, having my bladder stones removed a year ago this
past May...
the bug that came after me twice in 2 months was Pseudomonas. It is one of
the worst ones and both times
I had to be treated with IV antibiotics via a PICC line at home.

Again, you and I have discussed these ongoing UTIs both on list and off
list. There must
be some source, or culprit, causing your chronic infections. Mine was
bladder stones
but it has to be a process of elimination kind of deal. And only you know
your body best!

You already know that I have had repeated serious UTIs such as Pseudomonas.
Yes, it is very depressing not to mention life-threatening. And going
through an entire month
... how in the world did you let it go that long if it WAS so BAD? My
fevers that bad really quick
and I thought I would die before ibuprofen in some sort of antibiotic began
kicking in.

Then finding out that Pseudomonas has been growing in there the whole time?
 I would
began to think that your urologist OR even your regular physician... sits
around reading
comic books rather than trying to help you!

As far as how many times one can use the IV drugs before the bug gets
resistant to them
it is really academic if you NEED them anyway. My greatest fear with all
my troubles ALSO
was the fear that the bugs were growing more and more resistant. After
all...
when you have to resort to IV antibiotics... that is scary enough! There
is nothing left to resort to.
This is what leads me to think that your position does not give a rat's ass
about your well-being.
How you describe it... this is dead serious and something not to just brush
off again and again.

Also, like I have told you so many times... you might need to travel to get
a better urologist
and one really cares about your well-being! This quack that you keep going
to has not
been very helpful... has he? Therefore, because these things can get quite
serious...
you better get real proactive real fast. If I sound like I am yelling at
you or preaching
to you... maybe it is because that I care more about you than your doctor
does! Tough love
if you want to call it that. :-)

the tibia/fibia? I broke mine about seven weeks ago and there's been no sign
of healing. No attempt by orthopedics was made to reduce the fracture and
x-rays taken just last week have shown no bone growth and the separation of the
bones is still the same.

I've been pushing for some sort of surgery, but haven't had much success.
What's been your experiences?

meant to be a pain, or is she just not familiar with it? I'd like to help her

nursing homes are a drain on medicaid and frankly, they are often nightmarish.
The agencies have usually treated me well but I'm told they cost 4 times what
an independant costs medicaid. So why would states make their billing process
