Maybe try creating a new book that only contains the Chapter 4 files
and test in that. That should let you see if the problem is coming
from within Chapter 4 or from an earlier chapter.

This has been my experience, too. I find it terribly annoying because
I've designed my templates specifically with some paragraphs that are
blank.
Typing text into the paragraph makes the next paragraph feature work.
Leaving the paragraph empty simply repeats the current paragraph tag.

Your Figure number contains a second counter which you aren't
incrementing or displaying (< >). It's a shot in the dark, but what
happens if you remove the second counter?

got a book that includes landscape pages with different sizes than the standard
pages. Accordingly, the chapters containing the landscape pages are broken into
separate files. I'm working on the first chapter, Chapter 4

error is in the conversion filter, not FM itself. Apparently the
Japanese RTF filter is/was smarter than the standard RTF filter.

won't run in Mac OS X.
