FYI the fix is to check the "Disabled Physical Block Size Reporting" box in the
extent window. Note in testing this I had to delete the extent and create a new
one, toggling the switch on and then restarting the iscsi service didn't seem

Why not simply adding a new ovirt disk/direct lun and adding it to the existing
LVM group instead of extending the initial disk? This is a very common way to
achieve your goal.

If you used the default image for your self-hosted-engine, it will create the
disk of the size you specified, (50GB) but only allocate the space it actually
requires.
This means the remaining space on the allocated disk, is there, unused.

I have checked the installed version of VDSM:
vdsm.x86_64 4.50.3.4-1.el8 @centos-ovirt45
I also refreshed the repos and don't see any update for the VDSM. Looks
like that this version have not been released yet.
