I read the manual differently. My understanding is that ezmlm receives *all* messages and forwards a copy of those from unsubscribed posters (and only those) to the moderator who then accepts or rejects them. The mail manager software ezmlm sends to the list (a) all messages from subscribed users plus (b) those that have been accepted by the moderator. My interpretation is that the addition of headers, trailers or subject prefixes, as described at < http://www.ezmlm.org/manual/Modifying-Messages.html#Modifying-Messages>, happens at the time ezmlm sends messages to the list and would thus affect categories (a) and (b) above.
