It seems that the people successfully doing commercial VoIP are somewhat successful with the "flat rate per month per handset, we manage it all" model. Like $30 or $35 per month per phone, with a minimum of a dozen handsets per business or something like that.

With enough of those type of customers it can pay for a single staff position who can sit on top of an incoming ticket queue, whose job is to do things like create/delete extensions, IVRs, administer voicemail, etc.
