some of our users have been complaining about receiving duplicate e-mails
several days/weeks/months after they've been sent. They'll just suddenly
get another copy of an e-mail that they got two weeks or two months ago.
All the users experiencing this problem have their mail client set to NOT
delete e-mails from the server after they've been downloaded. Also, all the
users experiencing this that I've asked have been running either outlook or
outlook express (although about 90% of our users use one of these two if not
web mail).
This is a relatively common problem, and I too have only seen it with Outlook/OE. It appears to be a problem with Outlook, but I don't believe anyone has actually proven that. It can only occur when people have their messages left on the server.

It occurs when Outlook and IMail get out of sync (Outlook will store information about the last E-mail it received, but somehow either that gets changed or the IDs for the E-mails on the IMail server get changed).

I'm not aware of any resolution, other than to not keep the E-mails stored on the server (which will definitely fix the problem).
