Judi, I understand that Form = size + shape. Therefore "form" should be related to both isometric and allometric size effects. Shape is free of isometric size but allometric effects could remain. There is a "preform space" which is composed of all objects translated to a common origin, with size and orientation remaining to be separated.
I think there is something wrong in your last definition of "form", there should not be anything considered as "form" that does not have something of "size" within it.
