What version of the ARS? :-) This might be important to know as sometimes on a version of ARS that is not compatible with a higher DB version, you need to run the sp_dbcmptlevel stored proceedure to bring down the compatibility level of the database in order for ARS to work.. Joe

Thank you Joe and LJ, I made a copy of the ACTL SRS:SRC:AddAttachment which is called from the 'Add Attachment' button on the SRS:ServiceRequestConsole form. This original ACTL of SRS:SRC:AddAttachment works on both WUT and Web client on a Service Request which is based on a 'mapped' SRD option.
