sv_visiblemaxplayers is set by the admin plugin automatically according to

Run an strace on it and log it to a file. Then wait for it to crash.
You might want to also trying running it out of the vm/xen
We can't. If you want I can grant you access to check it yourself. It
doesn't do more then 4.

Not trying to give you a hard time, but if you want my opinion, it's
time to upgrade. I used AdminMod for many years, but started having
stability issues a year or two after it stopped being updated, so I
officially retired it. AMX Mod X really is the way to go. ;)

*strace* is a system call tracer, i.e. a debugging tool which prints out a
trace of all the system calls made by a another process/program
So find out the pid of the server and then run the following.
strace -p pid# -s 6 -f -t -o /tmp/strace.log
And when it crashes you will have a log.
If admin mod was still being updated, I would've used that when I got
back into running a server, simply because it's what I know. Granted, I
have no issues with amxmodx, as it works great also, I just would've
preferred sticking with what I know, if it wasn't for the lack of
updates. And I don't
When I tried it here more thank -fork 5 and it gets totally confused
about ports being allocated, so its likely he means this.

Hello,
I edited the motd.txt file and put in a standard welcome message and
other information (all plain text, no embedded HTML). I edited the
host.txt file to point to a jpeg file on our website, but it does not
show up in-game. All I see is an empty box where the banner should be.

Works fine on my setup currently at 69.65.59.239:27015 (plus forks). I
haven't yet figured out the exact dimension that is displayed in-game to
adjust the graphic used, but it is definitely loading up the graphic and
also spitting out the motd properly. My motd.txt is html, however.

I stuck with it as well because it was what I knew, and was reliable,

Thanks for letting me know. I logged into your server and got the same
result. I'm running L4D on Linux using Cedega, so it's no surprising to
run into little issues like this.
