I don't have kids and not much experience with them except with I was
a life guard and camp counselor.

That being said, I don't think teachers should be experts at
discipline or handling tough cases; I think they're supposed to be
experts at education.  That seems to me to be one of the problems:
parents outsource their problems to the school and expect the kids to
come home all better.

I'd be fine if teachers had the ability to say a kid wasn't
"teachable" and get them out of the school until the parents could fix
the problem.  If the parent couldn't there could be some other
institution to handle it.  Now I'm sure there's a bizillion problems
with that policy suggestion, but it comes to mind because I don't
schools or good kids should have to put up with someone else's
problems.

To me that's the personal responsibility area; if the parents can't
deliver kids ready to learn they forfeit their right to parenting. 
(again, wouldn't work in the real world, but it seems the beginning of
reform.)
