I think that was also written in the manual of the 70-200, but it is not
written in the manual on 100-400. The manual is btw a "manual" compared to
the "sheets" that come with most lenses. It has long lists of how the IS
is affected in all the EOS cameras, and it even goes to give exposure
compensations for EOS-1N RS and EOS 5/A2/A2E for using the 100-400 with
either EF1.4x or EF2x. That is quite strange as these cameras are metering
through the lens... but no mention about using any other focusing point
than the middle one.
But of course when 100-400 is used with the 1.4x, the aperture drops so
small that only middle AF point is active (with EOS 3 and EOS 1V)... so it
should have a different footnote. One that doesn't say "should be" but
says "can be"... ;-) And with 2x even the middle point is lost.
So, back to 70-200 f/2.8L...

Yes, I've too used this combo previously. No problems with any focusing
points. All work. How is the precision... haven't noted any differences.
Maybe there is, but again in real life.... well too little things to worry
about.
