i have this issue as well - but its been random.. and i have not found
what the trigger is... it happens on both my boa credit card and my
canadian bom credit card... very strange... i was going to report it
when i found a consistency / trigger to it :)

I copied the account and routing info from the 'new' account moneywell
created - to the old account i had already set up (in my case the old
account routing and account was blank)..
