We ran MT 8.1 against a Remedy 7.5 environment which worked ok for our
custom forms. We ran into a problem with the SRM Application where the
questions for SRD's were not getting displayed correctly for our users so
we had to switch back to our old mid tier servers.

We just spoke to our support organization and they told us Do Not install AR
System 8.1.01 if you downloaded it before today. They said that it will
corrupt your overlays. We were just about to do it and luckily we did not. BMC
apparently is pulling it off

I was at the RUG last month and in one of the upgrade sessions I recall a
presenter mention that all patches needed be installed before the upgrade.
We are on 7.6.04 sp2. Is it best practice to go ahead and install the
patches to sp5 (I believe) before the upgrade? Or just upgrade?

Creating a Form overlay does not immediately mean that you loose the
visibility on the base items.so, if the only changes you have are 5
custom fields..this is REALLY easy.
Upgrade your server to 8.1

I just tried to upgrade from 8.0 patch 002 to 8.1 GA on a Windows Server 2012 VM, I had no problem.I upgraded AI just after CMDB (no reboot).Though perhaps you should contact Rab (https://communities.bmc.com/communities/people/rab.boyce%40alderstone.com) it seems he had those kind of problems
