Re: [QUAD-L] New Bowel Management System - Peristeen Plus

2022-10-13 Thread Edward Tessier
I've been using Peristeen since it first came out. My traditional program of
suppositories and fleets gradually become more and more time consuming until it
lasted for hours, was painful, and triggered dysreflexia. it was also leaving
me with bowel accident so often I was afraid to leave the house. Peristeen has
been a complete game changer. It's not as messy so using it is easier on my
attendants. it does not trigger dysreflexia for me. it's not painful, unless
I have a hemorrhoid (regular hemorrhoid cream the night before is taking care
of that). it takes the predictable amount of time and is comparatively speedy
(about 45 minutes beginning to end every other day), so I can add it to my
morning routine without it messing up my busy schedule.

Perhaps most importantly, it is EFFECTIVE. It actually cleans me out and I
almost never have bowel mishaps, anymore. this has been a humongous benefit to
going out, making travel plans (It's very portable, fitting in a midsized
purse), my love life ( it not only eliminated untimely accidents it removed the
mood to killing anxiety of potential accidents), etc. I hadn't really
realized how my old bowel management program had compromised my quality of life
until I saw the difference Peristeen made. In fact, I would say it was at the
very top of the list of the products that have made my quadriplegia manageable,
along with a Sure Hands lift, a Ride wheelchair cushion, accessible minivan,
and Alexa as a smart home controller.

The only downside I experienced with Peristeen was its cost. Since it wasn't
covered initially by my insurance, I had to pay out-of-pocket around $300 A
MONTH. This was rough to handle but since it dramatically increased the time I
had for work, and allowed me to go to the office without racing home from bowel
accidents, it was worth it financially. it took a few years but eventually I
found a doctor in my network (Kaiser Permanente) that believed in the benefits
of Peristeen to advocate for it internally. Late last year, my insurance
decided to cover it completely.

If Peristeen had not come out when it did, I probably would've ended up with a
colostomy bag. I understand they can work wonders for some people, but I was
dreading it. Peristeen gave me options, and a huge boost to my quality of
life. I rave about it to every Quad and Para I meet. my wife jokes that I'm
trying to get a job at Coloplast (the manufacturer).

I've been happy to see that Peristeen is is being strongly supported by
Coloplast. they just came out with an upgraded design that makes the reusable
pump last longer.

if anybody has more questions about it, please reach out.

Subject: [QUAD-L] New Bowel Management System - Peristeen Plus

Hi everyone!

I've been a c5 quad for 40 years now. Bowel programs are the most difficult
thing for most of us. Grr
Recently I was at a conference and talked to the Coloplast rep regarding their
new(ish) bowel product, Peristeen Plus. It's an enema system (nothing new
there), but the design of it and the way it works seems like a real
improvement. Check out the YouTube video.

Disclaimer: I have not tried this product. I don't work for Coloplast or have
any affiliation with them.

Peristeen Plus is an effective alternative to conservative bowel management
methods. It can be administered either on your own or with assistance, and
enables you to decide when and where you want to empty your bowel. Used
routinely, daily or every other day, it can help reduce the physical discomfort
and worry that fecal incontinence and constipation bring.
When the recommended amount of water is introduced into the bowel, water and
stool are emptied into the toilet.
Peristeen Plus can be used by a wide range of people with neurogenic bowel
dysfunction (NBD), from children to adults. The patient must receive thorough
instructions from a qualified healthcare professional before using this product.

I would consider contacting Medicare/Medicaid and tell them to stop making
payments to your provider and then contact Invacare-Customer Care, with the
 serial number of your chair.

So its starting all over.

The name of the chair place I get from my insurance.1 star rated, very bad

Why not call Invacare directly and get the story from them. Tell them what you
want and why you want to. Let them pull while you are pushing. BTW, who's the
customer anyway? Customer, is always right. Call Medicare/Medicaid in your

footrests instead of the platform plate for all 3 chairs. I’ve been in an
Invacare Action Arrow Storm for 19 years. They no longer make this chair but
the Torque 3 is the same chair, different motor.

I called vendor to tell them to order the Invacare but they told me we have to
provide reasons why the other chairs won’t work before the FDA will sign off on
it (since Invacare is currently under Consent Decree with FDA). So they still
want me to demo chairs and come up with reasons they won’t work.

I am so frustrated. I know one deal breaker is the armrests because I have a
bag that we attach to a particular spot on my armrest and I put my leg bag in
it so it is hidden. Their suggestion is to put in on front of seat pan. That
won’t work, not even gonna try it, what’s not broke doesn’t need fixed.

Also, I believe by having the same base I won’t have to move e-z locks in the
floor of my van where as if the other bases are different not only will I have
to pay to have bolt added to chair but the expense of moving the bases.

Anyone else want to help me come up with reasons why other chairs won’t work?

My rear is the one that sits in it so why can’t I just pick what I freaking
want?

Nicki

I’m getting my 3rd chair in 19 years. I have had the Invacare Action Arrow
Storm for all 19 years. Invacare got in trouble with the FDA and are under a
consent decree with them. My DME is really trying to steer me away from getting
an Invacare chair. It would be a Torque 3. I am suppose to be testing it as
well as a Permobil C350 and a Quantum Rival. The provider brought out the
Quantum on Friday and it has a plate footrest (I have always had 2 footrest
which allows me to straddle table posts at restaurants) I don’t even think my
feet would stay on a center mounted foot plate. They were suppose to bring the
Invacare first and the employee agreed he didn’t think my feet would stay on
the plate. He even told me that the motors in the Rival go out but I didn’t
hear that from him. The DME supplier is trying to get footrests for the demo
chairs but said they can only get 1 side for the Invacare.

Would love input on any of these chairs.

Danny,

Do you drive and use the EZ-Lock to lock into your van? If so did you have to
move driving equipment and EZ-Lock to compensate changing from an Invacare to
the Quantum?

My Invacare base is 33” and the Quantum is 36” so this is a concern especially
because it will be out of pocket to add new EZ-Lock to chair and if I have to
make changes in my van the expense will be greater.

Why not call Invacare directly and get the story from them. Tell them
what you want and why you want to. Let them pull while you are pushing. BTW,
who's the customer anyway? Customer, is always right. Call
Medicare/Medicaid in your state and let them know what the provider is doing

You know how when someone is in a car accident they often claim a neck injury,
which is nondescript and difficult to pinpoint.  You just have trouble sitting
up all day in those chairs.  You don't understand why, but it hurts and you
have to go back to bed.  Only the Invacare seems to allow for your sitting up
all day.

Need your opinions and input please

I have been injured almost 19 years (Thought it was 20 but I was wrong) and I
am getting ready to get a new chair.

This will be my 3rd chair and and my 1st two were Invacare Action Arrow Storms.
They are no longer making this chair :(

I hate change! I went to an evaluation appointment and was told the chair I
want (Invacare Torque 3) is considered a Group 4 chair

And it would be difficult to get. They recommended a Permobil C350 or Quantum
Rival.

They told me that an Invacare plant had been shut down by the FDA ( I think
that’s right) and their concern is if I got the

Invacare they might quit making parts for it.

I feel they probably make more $$ off the other two chairs (I just really have
trust issues with them)

When you call Invacare Customer Service, be prepared to lay it on thick. Let
them know you love their chair and would not want to be stuck in another brand,
start crying if you must. Let them know what the vendors are doing to
consumers. Who knows what they can do for you. Good Luck and I hope you

My last 2 chairs have been invacare storm series also with rear wheel drive, I
just had to get a new chair and wanted the same, since they work for us plus
fit in our mini astro wheelchair van..They told me everyone including
invacare is getting away from the rear wheel drive chairs, so my supplier
ordered me a chair from Pride Mobility that still makes a rear wheel drive,
They use Quantam Electronics I think. So far I am very happy with this new
chair except my legs are so long that I don't have very much clearance from the
floor...so I have to recline back a few inches when going over ramps, thresholds
or outside, I'm now used to that but most people would not have this problem
probably. I love the Electronics...they have a quick change button that can
switch from tilt/leg lift or back into drive and then 4 speeds of drive, this
chair is fast goes 7.5 mph. It even has a built in clock and how many miles you
driveSince I got it in February I have put 91.3 miles on it. The tires are
extremely good and show zero wear so far.( my invacare tires would be at
least a quarter of the way worn out already) so these must be very well made.
I now have my last chair going as a back up but my old back up chair I am
giving away if anyone wants it or needs partsIt works and has the charger
with it but has set so long It would need the batteries replaced...It is also
and invacare storm rear wheel drive chair with tilt and leg lift, there is a
place online called Wholesare batteries that sells all the same batteries we
need for much, much cheaper than the suppliers do. Dan H. St. Louis, Mo. area.

Like I was saying earlier, you have to basically live in the chair for who
knows how many years.  You should be making that choice.  I'm in a Pride chair
now, after two Invacares.  If I hadn't been steered away from Invacare, that's
what I would have.  I don't like this Pride chair.  Would I be happier in an
Invacare?  I don't know but it would've been completely my decision without any
steering by others.

Re: [QUAD-L] New Chair

2017-06-19 Thread Danny Hearn
Dan H. St. Louis, Mo. area.

Re: [QUAD-L] New Chair

2017-01-19 Thread RONALD L PRACHT
Ive gotten two manual chairs since 1998. My first one was an action mvp and the
second and current chair is a quickie 2 which I have held together from ebay
parts from (2001-2017). I have replaced everything on the chair at least once
except the red frame, which is wobbly because its a folder. Ive had 10 sets on
rear tires, 4 sets of casters, 2 jay backs, 3 jay 2 cushions, 8 sets of scissor
brakes, 6 sets of sideguards 7 seatbelts, 4 seat platforms, I found every part
either at sportaid, ebay or somewhere else online. In 2001 I got lucky and was
presented a brand new p-222 special edition Darryl Gywnn Racing power chair.
That chair has some guts, it will climb places you shouldn't go. The first week
I had it I took it close to a mile on rough grassy hills to get to the lake and
popped a front tire. I was stupid and got air filled front casters(8x2) and to
make it worse you couldn't take the wheel off without taking the tension off
this spring system used for the suspension. I  called the dealer that presented
it to me and he said, I learned the hard way with a chevy nova not to take
things apart I couldn't handle, he was telling me not to get my father to fix
it, but the price was 180.00, so my dad pulled it apart in the living room and
I replaced the air filled tires with foam filled. That chair now sits in the
corner with dead batteries and Im still in my quickie 2.
The biggest problem how things are handled with mobility solutions for the
disabled is inequality. One guy gets a house given to him, free vans and
another guy is given one hospital chair. Nothing is equal or even close to
fair, the squeaky wheel and the best insurance plans get the most and the best.
We need something that's equal across the board for every disabled individual.
Im so tired of one golden boy or girl getting everything while someone else is
laying in a bed that they cant get a new mattress for with half a dozen bed
sores. Am I the only one that's heard the stories and had disabled friends die
over a wheelchair cushion they never got?

Ditto as Danny as far as the reclining option on my Permobil M 300 in
September 2011 and the price of the entire chair was $32,000 I think.
Medicare covered that but I had to pay the 20% that Medicare did not pay
and had to pay the full $5000 for the recline option.

I finely started the new chair process. I just kept putting it off. Cost to
darn much, I owe 20%. They told me Medicare no longer pays for seat elevator
or faster motors. So that's even more for me. I have AARP Medicare
Replacement. Covers a bit more, but same rules for durable medical.

Was anyone able to get seat elevator or faster motors covered? If so, what
need did you use?

I never tie down, I sit sideways behind the seat. Now my motors auto break
is so warn out, I roll a lot. We had to hit the breaks hard coming home. I
was too much at an angle and rolled into the back of the seat. Snapped my
joystick right off. All the screws just snapped. Luckily I had 1 extra screw
at home. Supposed to use 4.

The new chairs sound pretty good. Said it could have a USB, and the joystick
can operate remote controls. Very cool.

New Law Expands Access To Special Needs Trustsby Michelle Diament | December
16, 2016
Tucked inside a larger bill signed by President Barack Obama this week is a
tweak to federal law that’s designed to make it easier for people with
disabilities to save money.
Under the new law, individuals with disabilities can create a special needs
trust for themselves rather than relying on others to do so.
Previously, such trusts needed to be established by a parent, grandparent,
legal guardian or a court.
The change outlined in a bill known as the Special Needs Trust Fairness Act was
approved unanimously by the U.S. Senate last year and passed the House of
Representatives in September.
But due to some differences in the House version, the measure returned to the
Senate and was ultimately bundled within the 21st Century Cures Act, which
Obama signed Tuesday.
The option for people with disabilities to create their own special needs
trusts takes effect immediately.
“This provision replaces an antiquated law that was unfair and outmoded in its
treatment of people with disabilities,” said Sen. Chuck Grassley, R-Iowa, who
sponsored the Special Needs Trust Fairness Act.
“Those who want and need to set up a trust to help pay for their care should be
able to do so, plain and simple,” Grassley said. “This measure allows
individuals to act in their own interests with their own assets without having
to rely on a family member or the courts.”

Everybody that lives as a quad has a different situation. The most important
factor is a support system. Some of us have tons of support and some have very
little. In any case we all still have life and we try to do the best we can
with what we have left. The guy with the crushed legs is no different, he is
doing the best with what he has. Life is for the living and we all are still
among the living striving to make the best outcome we have. Is a blind person
worse off than a quad? Is a person with aids better off than a para? Who really
knows, we are just a group of quads here trying to help each other with one
issue at a time. Ive had great times as a quad and horrible. At this moment Im
in pain and trying not to take more pills than I need to. I did complete my
power cycle this week and Im going to go on a big adventure this week
regardless. Im trying to find good still as often as I can.

The biggest thing people or the movies don't get is that not walking is the
least of a paralyzed individuals problems and everybody isn't paralyzed from
the waist down. Most also believe that people that can move their arms a little
are not quads. The movie is fun to watch but doesn't show any of the
nitty-gritty rubber glove work that's involved with quadriplegia and that's ok
because very few would watch a real life quad movie. People don't want to know
the truth.

You are 100% correct and that is why I added all the extra information that
I did. I just forgot to mention the CMS requirement. Thanks for adding all
that extra info. It would never work for me or many of the quads on this
list. I had just never seen a foldable POWER chair and perhaps list members
might know of Paras or others who might benefit for such a chair but,
again, where would one go for repairs?

It is not a very complex piece of equipment so anyone who works on cars or
other motorized vehicles (like my great-nephew) or engineer could make
repairs.

Nice ultra light travel chair for anyone living in a freedom or independent
living center, with flat dry surfaces. The travel chair is not coded for
Medicaid or Medicare.(CMS) This would be a great means of transportation
for those who are elder, but have issues walking long distances. A means of
transportation that is not coded, will not be honored for payment via CMS.
I don't believe this travel chair is rated as a rehab chair, nor do I
believe this chair can be altered or modified to meet the complex rehab chair
requirements/ standards. And should the device (travel chair) fail to perform
or cause injury to its user... who carries the liable insurance on this
product.

It truly scares me to think of a someone in need of a rehab or complex
rehab device, would consider the reference to safely transport them from point
A to point B. I am sure that I look hard I might find a Quad that would
find some comfort in using this light travel chair, but I don't see it for
the greater Quad population. Also, the manufacture claims that they offer a
1 year warranty. Should that warranty be used, who exactly would travel
from China to work on the chair? This is important.

I wish it were that easy for me, I cannot try out a new chair unless it’s
equipped with chin control or sip and puff and nobody in this area has such an
animal! But to be truthful, I really like my Omega Trac and am willing to pay
for the repairs to it if we cannot repair ourselves. I bought it over 10 years
ago from Ebay and it was in Hong Kong. I had it shipped to Texas and
electronics installed. Because I was not approved for disability and had to
wait until I turned 65 to get insurance I always bought my own chairs and had
to buy them used because I could not afford a new one. We bought two other
omega tracs for parts, and have managed to keep this going. Before the hour
meter broke I had 900 hours on the meter and that was four years ago. I have
looked at other wheelchairs, primarily Permobile but they do not allow me the
versatility and safety aspects of the omega trac. That’s saying that I will
probably stay with this chair until I die. J J I should probably get a chair
in case this one dies permanently, but me thinks I’ll cross that bridge when I
get to it. Good luck in finding a chair, and be sure to listen to all the
excellent advice the others on this forum have.

I have yet to meet an old electric wheelchair that could not be repaired or
restored. My advise it to contact Medicare/Medicaid (CMS) in your state
for the updated procedure of obtaining a new chair.

Yes, it begins with a face to face with your doctor. Hopefully a MD that
specializes in Rehab Medicine.
Yes, a face to face with a licensed Occupational Therapist.
Yes, a doctors order for new chair.
Depending on your physical condition, your medical needs and finding a
manufacturer that can meet those goals, it can take several months.

RE: [QUAD-L] New wound

Steve

I have been battling a wound in the groin area for nine years now, and have
just decided to live with it. Surgery was an option several years ago, but
because it is in an area that has so many nerves I could not find a surgeon
that wanted to take the risk. It will almost close up but then it opens up
again. I did do all of the proper things like six months in bed, LOL-that
wasn’t fun. The wound vacuum would not adher, so we finally just gave up and I
go into the wound clinic once a month just to be monitored and get supplies.
The older I get the more fragile my skin becomes. So I figure I will take it
to my grave. (not my grave as I donated my body to the university medical
school, so they will get to check it out) my wound does not get any better if I
stay of my chair, so I am fortunate that I can still stay in the chair for the
15 hours IM up. I’m much more comfortable in my chair than in bed.

are at least stage 3 or worse. Even though my wound is only stage 2, it just
won't completely heal. I have discussed the possibility of surgery, i.e. skin
flap, to repair the affected area, but he thinks any risk of the surgery not
healing properly outweighs just living with the problem I have now.

It gets so old staying out of my chair for extended days at a time, but staying

Here's what I know about treating small wounds. Start by treating with Dakin's
solution, you can make it yourself. It consists of 21 parts sterile water one
part Clorox bleach and a pinch of baking soda. This is much cheaper than buying
it and it is the actual solution. Since you already have aQUACEL, be sure that
they just lay it in the world and not pack the wound. It should be very fluffy
and not tightly packed. Over packing wounds delays healing and often causes
more damage. Cover the wound and try to stay off it as much as possible.
True Dakin's solution is 7 parts water to one part bleach with a pinch of
baking soda. This is often too strong to be used at home. Medicaid won't cover
it because it is easy to make.Best Wishes, John s.

bone cut out. came out ofsurgery with skin flap and 40 staples up to my knee on
the back of my leg. staples and stitches are coming out wednesday...6 weeks
bedrest. then hopefully all this will be over. before surgery they also
suggested oxygen chamber treatments. i think that may help with small wounds.
i was exhausted trying to get it healed, the spongy dead piece of bone had to
come out anyway, so they closed it.

My wound is only stage 2 at worst. I think wound VACs are used on wounds that
are at least stage 3 or worse. Even though my wound is only stage 2, it just
won't completely heal. I have discussed the possibility of surgery, i.e. skin
flap, to repair the affected area, but he thinks any risk of the surgery not
healing properly outweighs just living with the problem I have now.

It gets so old staying out of my chair for extended days at a time, but staying
in it long-term definitely makes matters worse.

My wound Dr. has been treating my wound with Aquacel AG for more than a year

and it appears my wound is not nearly as deep as yours. In fact, he just
recently switched me to a different type of bandage called Endoform dermal

wound. So far, it hasn't done much better. Keep in mind that this is in spite

I asked about the oxygen chamber.

it is the anacet not covered. 75 bucks per tube. don't know ounces...not home
yet.

Medicaid won't cover
it because it is easy to make.
Best Wishes,
John s.

Very sorry to hear that Bobbie. I have an Invacare alternating
pressure/low air loss mattress. It works very well as far as pressure
relief, but the pump is kind of loud. So, I disable the alternating
pressure part and just use Low air loss. Prior to this one I had one
called the Genesis Deluxe 8-inch Alternating Pressure and Low Air Loss
Mattress System, it was quieter, but only lasted about 2 years.

I need a new chair pretty darn soon. Mine is getting run down. The auto
brake barely holds me in place. I usually don't tie down in the van. I just
pull in sideways and get tight behind the front seats. But if at all facing
forward, when the van stops, I roll forward. While in for something else,
the repair shop said the motors are going bad and said I might not be able
to get those because they are too old. When I recline, it makes a loud
snap/clank noise like metal is braking and the seat jerks. I can't count
how many small nuts and bolts I have found on the floor and can't find where
 they fell off.

I've always had front/mid wheel drive, except my 1st chair. 21st Century,
large rear bike size wheels. Very fast, but ugly and not real maneuverable.
They still make the exact belt drive chair. Fun chair, but hard to get in
mini van, no turning around inside.

Other than that old fashion one. Looks like the fastest now is 8.5. I have
never tried a rear wheel drive, with the smaller rear wheels. Do they turn
sharp? These mid wheels can turn around in space. Can I spin around in my
 van?

 Permobil M300 - 7.5mph mid
 Quickie QM-720 - 8mph mid
 Quickue P-222 - 8.5mph rear

I'm looking into these ones. Any thoughts?

The only issue regarding new motors is replacing the control module too.
But that is not a major issue if insurance is paying for the work and
parts. The parts are usually in stock and ship in 14 days. Labor is about
2-2-1/2 hours max.

I have a Permobil M300, the fastest I can get it to go is 5.5 mph. I
tried it out on the flight deck of the USS midway while on vacation.
Which is fine with me, the terrain I have to go over outside of my
home is usually pretty rough. The only problems I've had is one of
the armrests falling apart, but I think it is normal wear and tear.

yes, bobbie sent me a video where her chair comes with a power headrest.(same
chair) the rep said it wasn't available nor a medical necessity. that medicaid
wouldn't permit it. i proceeded to tell her i couldn't unbolt it. it's a
cosmetic issue, but sometimes it would be nice to take it off. or should i say,
hit a button to put it down. it falls in the category of a para not wanting a
power chair. but NONE of the options were discussed with me except lateral
support that HAD to be changed. yes, derrick did a lot of research, it really
isn't our responsibilitythey should know but we're the ones getting the
chairs. in all respect, my rep said she'd come out until it's right.
dianna (still need shorter legs or foot rests on mine. using dish towels to get
my legs higher...not cool)

Well I don't really doubt what dianna said because wheelchair companies are
supposed to know when a person in your shape needs and work your insurance
company will pay for it shouldn't be our job to know what we need I mean we
wouldn't even know if our chairs could have certain things done to them

my wheelchair rep came out today and changed the back on my chair. i'm so
happy, feels great. it's not a jay3 but a veralite icon air back. it also has
little lateral support. the other had way too much. she was familiar with the
jay3 website you showed me but my computer was malfunctioning and I couldn't
get aol up running to get to your links for you tube but i'm going to send them
to her later. thank you sooo much for feedback. she's also coming back out to
change my legs/foot rests as they are still too long after changing the back.

chair. They should not get away with this.
Until you find a company that really cares and takes the time to

Then ask for a demo to try for a few weeks. Read reviews, ask other
users and give it time to adjust for the change. A new different chair
 takes time to adjust to.

I do not think wheelchair manufactures and DME dealers consider people
sitting in wheelchairs for 12 hours a day. They should spend the time in
customers shoes and ride around town and into stores to see what its like.

DME are sparse in rural areas like I live in so there is not many
choices. My friend down the road was just told to find another dealer
after getting his new wheelchair just because he told them and argued
that he never got a chest strap. A chest strap is not much $ but they
 could care less.

Its just another hosing that the disabled get. Here's your wheelchair,

Med-i-dont-care for not paying for pressure mapping and for better

sign the required paperwork until you get exactly what you need. Remember that
providers are guided by you requirements and needs. They must comply with
insurance perimeters and CMS codes. Learn as much about the manufacturer via
the internet. You can try out anything you want. Manufacture Reps are more
than willing. Take Charge of YOUR Life. Otherwise others will.

All companies the same. They do not give a crap about fitting the best

Until you find a company that really cares and takes the time to
evaluate you, You have to really spend the time to research your self.

Here's your wheelchair,
cushion or van, now go away until you can order a new one. Plus, I blame
Med-i-dont-care for not paying for pressure mapping and for better
equipment.

I hope you are working with a seating and positioning specialist. Chair
bases are all similar, but the seating and positioning is paramount.
Wheelchair Reps are not seating and positioning experts. A bad seat will cost
you
and the payee lots of money over the next decade, if not done properly.
