One other thing to keep in mind is that, if you do not have a system voice set, or if you have system speech turned off for whatever reason, you may not hear anything, and the app will launch in the background. It just occurred to me that this might be the problem with thinking it's not open.

That's weird, because version 2 doesn't support 1080P. DO you have Xcode on your mac? And, apparently, your mac is responding to the remote, which means you have an IR port somewhere on it. You need to pair the remote with the apple TV so it will only respond to that, rather than the computer.

No, iTunes doesn't need to be open to play content. There's a setting in the remote app that lets you maintain a connection to which ever thing you're connected to. Also, once you have the remote app connected to your apple tv, there's a control tab, that will let you actually control the ATV
