This can happen whenever the volume on the iPhone is set to 100%
actually not exactly 100% but between 99.5 and 100 but thats out of
your control).
It should help to decrease the iOS volume outside iPeng by one click.

Looks like an issue with the artwork cache or you are using some image
format iOS doesnt support. Or you have specifically set no image for
that track.
The image for the NowPlaying screen is an unscaled track image, the
Playlist uses the album image first and then updates for the track image
In iPeng you can customize the whole main menu and move it to Radios
through that.
On iPhone, use the Edit-Button in the More... menu, on iPad you can
reveal it by pulling down the main menu.
You can then re-order using drag

The TIDAL menu is provided by the server and configuring it is a
per-player-setting. You can configure this on the MySqueezebox.com web
site.
One thing that often can cause issues is if SqueezeLite for some reason
has a wrong MAC address set.

Currently the font size is fixed, iPeng 9 was developed long before
dynamic type was a thing and changing all that is a huge amount of work.

Apple at least claims that the ipa's are signed per AppleID but I'm not
sure to which extent that's true.
People used to jailbreak their devices to install ipa's themselves, and
I think those are typically cracked, so there must be some sort of copy
protection.

most old iOS versions have either a purchases tab under Updates or a
first item purchases (again, under updates.
I can no longer get my iOS 3 devices to connect to the App Store but
they do throw an error and my iOS 6
